Day Camp Forms

To comply with the state of Maryland’s Youth Camp Standards, the Department of Parks and Recreation requires certain information and forms from its participants. It is the parent/guardian’s responsibility to provide the answers to the below questions and deliver all completed forms to each program location by the June 1 deadline. Children may not attend any session without completed information and forms on site. NO EXCEPTIONS.

Required Summer Programs Participant Information

We require all Summer Program participants to provide us with answers to a few basic questions to protect their health and safety. Beginning February 15th, there will be an Activity Section available on the PARKS DIRECT registration system to collect this information. Simply register your child for the section titled “Summer Program Participant Information” (16600-300A) and answer the required questions. Children may not attend any session without completing this information. These include:

  • Emergency Contact Name (other than parent/guardian)
    • Please provide the full name of the person to contact in case of an emergency.
  • Emergency Contact Phone Number (other than parent/guardian)
    • Please provide the phone number of the emergency contact.
  • Physician's Name
    • Please provide the name of your child's physician.
  • Physician's Phone Number
    • Please provide the physician's phone number.
  • Is the participant a swimmer/non-swimmer? 
    • Please indicate whether you want your child to take the Swim Test or be designated as a Non-Swimmer. (See page 4 of the camp brochure or online flip book.)
  • Are there any health issues? 
    • Please answer Yes or No. Participants who have health issues such as allergies or other illness, take medication (prescription or over-the-counter), or use emergency medical devices such as inhalers or Epi-pens should answer Yes.
    • The Health-Medication Form will be printed with your Day Camp Confirmation Receipt at the end of the registration. This form must be completed and returned to staff prior to your child's participation in the day camp program. If you answer No, you will not need to complete any additional health forms.
  • Has the participant been immunized?
    • Yes - student in a Maryland public or private school
      • If you answer Yes - and the student was in a Maryland public or private school, you will not need to complete a Maryland DHM Immunization Certificate.
    • Yes - student is home-schooled or was in a school other than in the state of Maryland school
    • No
      • If you answer No, or Yes, the student is home-schooled or was in a school other than the state of Maryland, then the Maryland DHM Immunization Certificate will print out with your Day Camp Confirmation receipt. This certificate must be completed and returned to staff prior to your child's participation in the day camp program.
  • Does your camper need assistance with applying "SPRAY-ON" sunscreen during program hours or outdoor trip activities?
    • Please indicate whether your child will need assistance from staff when applying spray-on sunscreen during program hours or outdoor trip.

  When you've completed this information once, and if you want to register the child for additional camp sessions during the summer, these fields will automatically populate with your answers.

Day Camp Forms

Once you have completed the required participant information, you will be directed to our forms page where you can download and complete the forms they’ll need in order to participate this summer. A complete set of forms must be submitted to each program location your child is attending before June 1st. Children may not attend any session without turning in these forms.

Participant Profile Form
The Participant Profile Form (PDF) includes emergency contact information and general information about the participant. It will be signed on the first day of camp as you drop off your child.

Day Camp Authorization Form
The Day Camp Authorization Form (PDF) details who is allowed to sign-in and sign-out your child from Day Camp. It is required for every, day camp participant. A completed copy must be turned in to every program location prior to June 1. The form is also available in Spanish (PDF).

Health/Medication Form
The Health/Medication Form (PDF) includes detailed information about a participant’s health issue, including illnesses, allergies, and any medication, emergency medical devices, or asthma inhalers that are taken during camp hours. If necessary, please return a completed copy to every program location prior to June 1.

Maryland Department of Health Immunization Certificate
The Health Immunization Certificate Form (PDF) details the participant’s immunization history. It should be completed for any child who did not attend a public/private school in Prince George’s County. If necessary, please return a completed copy to every program location prior to June 1.

Sunscreen Application Consent Form
The Sunscreen Application Consent Form (PDF) obtains permission for program staff to assist in applying spray-on sunscreen. If necessary, please return a completed copy to every program location prior to June 1.

Participant Electronic and Communication Devices Policy and Waiver
The Participant Electronic and Communication Devices Policy and Waiver (PDF) details the Department of Parks and Recreation’s policy as it pertains to participants bringing electronic/communication devices to program locations. It is required for every Summer program participant. A completed copy must be turned in to every program location prior to June 1.

Participation Assessment Form
The Participation Assessment Form (PDF) is ONLY required for those participants requesting disability support in an inclusive camp or for Therapeutic Recreation Camps. If necessary, please email a completed copy to program.access@pgparks.com prior to June 1.
